Does Resveratrol Live Up to the Hype?

Peter Attia and David Sinclair, two of the most recognised names in longevity medicine, hold opposite views on whether resveratrol supports longevity. Attia has publicly dismissed its longevity value, citing the failed replication of the sirtuin research behind it (1). Sinclair, the Harvard geneticist who put resveratrol on the map, still takes about a gram every morning himself (2). Resveratrol has also been studied for other health outcomes, and here we look at what the evidence does and doesn't support, and why.

What Happened to the Longevity Claim?

The resveratrol story began in 2003, when Sinclair's lab reported it extended yeast lifespan by activating a sirtuin enzyme linked to stress resistance and calorie restriction. In humans, this enzyme is known as SIRT1, central to the resveratrol story that followed (3).

The idea carried into mammals in 2006, when resveratrol improved survival in mice on a high-calorie diet, cutting mortality by around a third, though those mice also had diet-induced fatty liver disease, so the benefit may have been protection from that damage rather than slower ageing (4).

The follow-up evidence hasn't held up. On an ordinary diet, resveratrol improved some markers of healthy ageing in mice but didn't extend lifespan (5). The underlying SIRT1 mechanism was also called into question, when the effect turned out to depend on how the lab test was designed rather than a real biological effect (6). A more rigorous, multi-site government-run trial confirmed the negative lifespan result, even though a comparison drug tested alongside it did extend lifespan (7).

In humans, one of the strongest long-term studies found no link between natural resveratrol exposure and mortality, cardiovascular disease, cancer, or inflammation in nearly 800 older adults over nine years, though it measured diet rather than supplementation (8). Put together, the mechanism remains only partly confirmed in humans, and the strongest data don't support a longevity benefit.

What Do Human Trials Show in Other Areas?

Separate from longevity, resveratrol has been tested against specific health markers.

  • Cardiovascular health: A grape extract supplement containing just 8 mg of resveratrol alongside other polyphenols reduced oxidised low-density lipoprotein (LDL) and apolipoprotein B (ApoB) in statin-treated cardiovascular risk patients over six months (9); a separate 30-day trial in obese men found metabolic improvements, including insulin sensitivity (10). Blood vessel results are mixed: 270 mg improved flow-mediated dilation, a vessel-health marker, in overweight or hypertensive adults, but 150 mg/day for four weeks had no effect on endothelial function or inflammation (11).
  • Type 2 diabetes and cardio-metabolic risk: A meta-analysis of 17 trials in 1,337 people with type 2 diabetes found resveratrol among the better-performing options for insulin resistance, lipid profile, and blood pressure, though rankings varied by outcome (13). A separate 24-week trial of 200 mg daily reduced HbA1c, fasting insulin, HOMA-IR, CRP, and IL-6, with no change in cholesterol or triglycerides (14). Another trial in people with well-controlled type 2 diabetes found no improvement in insulin sensitivity or intrahepatic lipid, though a metabolite correlated with metformin dose, suggesting metformin affects its metabolism (15).
  • Bone health: A randomised trial found 75 mg of resveratrol twice daily improved bone mineral density at the lumbar spine and femoral neck in postmenopausal women over 12 months (16).
  • Polycystic ovary syndrome (PCOS/PMOS): Small trials report improvements in some hormone and inflammatory markers, though the evidence remains limited (17, 18).

Why Absorption Still Matters

Resveratrol's low bioavailability is a recurring limitation across these studies. It absorbs well, but the body quickly breaks most of it down, leaving blood levels far below what lab studies use to see an effect (19). That gap is what delivery formats like liposomal encapsulation are designed to address.

What About Safety?

At 100 to 1,000 mg per day, resveratrol is generally well tolerated, though gastrointestinal side effects become more common at higher doses.

  • Liver enzymes: At 1,000 mg daily for four weeks, it altered several liver enzymes in healthy volunteers, worth noting for drug interactions (20).
  • Blood thinners: In rats, it increased warfarin exposure and its anticoagulant effect, a possible concern for people on blood thinners, though unconfirmed in humans (21).
  • Kidney function: Problems appeared only in a cancer trial using roughly 5 g daily, far above typical doses and in patients already vulnerable to kidney impairment (22).

The Bottom Line

Resveratrol's longevity claim doesn't hold up under scrutiny: the science that started it hasn't translated into proven human benefit. The more promising human trial support lies elsewhere, in cardiovascular risk markers, type 2 diabetes, PCOS/PMOS, and bone density, though findings are not consistent across every study. None of these are longevity outcomes on their own, but managing them is part of what living a longer, healthier life involves, and that is where the current evidence points.
